Beneficiaries. The membership form includes a segment for designating your insurance benefict benficiaries. These fields must be completed. If you don't wish to designate beneficiaries, type "n/a." If you have already designated beneficiaries and don't wish to change them, type "n/a." Remember, if you don't designate beneficiaries, your benefits will be paid to your estate.
